Self & Family Managed Care (SFMC)

What is it? In Manitoba, the Regional Health Authority (RHA) in your area provides community-based support and services to eligible individuals who require assistance with activities of daily living (ADLs). As assessment will be done by an RHA Coordinator, who will determine the client’s individual needs, existing supports, and community resources; which will then determine the eligibility for home care, the type and the amount of services that one will receive.

 

In SFMC, funds are provided to a self or family “manager” (usually child of the client) to arrange services privately in liew of RHA home care services that they would otherwise receive from community home care. These services (funding amount) are based on the assessment that was done by the home care coordinator. A manage then has 2 options:

 

1. Act as an employer – recruit, hire, train, schedule and manage your own staff. This option makes the manager responsible for payroll, payroll deductions, and submitting them to the Canada Revenue Association (CRA). This option is very time consuming and intensive.

 

2. Hire Daughter On Call – we will take the responsibility as acting as an employer. We provide trained staff, handle the scheduling, and all of the other ‘headaches’! You simply pay us with the funds you receive from the RHA.

How Do We Apply?

To apply, call your local RHA Home Care Office. Ask for an application form for SFMC; and if an assessment has not been done recently by an RHA Home Care Coordinator, then request an appointment be arranged for one.
